Invitation to attend Stakeholder Workshop

Tuesday 12 July 2016 5.30pm – 8.00pm

The Academy (Lower Ground Floor),

The Great Western Hospital (light refreshments will be provided)

Last November, GWH started the process of a review of specialist services for people with diabetes and are now developing a new model of care.

This event is for everyone with an interest in diabetes care locally, whether as a patient, a carer, a clinician/professional or someone who supports people with diabetes.

This event is your opportunity to:

• Meet together as patients, carers and clinicians to update on the progress since last year and the development of ideas around new models of care

• Give us your feedback on the right care for Type 1 & Type 2 diabetics including care in the right place at the right time

• Focus on care in the hospital as part of the development of integrated specialist diabetes care

• Improve the experience of patients. This is the start of the conversation about how we develop services which meet the needs of patients, families and carers for the future.

If you would like to attend then please confirm your attendance with:
