After the publication of Building the Right Support NHS England, the Local Government Association (LGA) and the Association of Directors of Adult Social Services (ADASS) supported the creation of 48 Transforming Care Partnerships (TCPs).

Each of those 48 TCPs have been working on their plans to change services in a way that will make a real difference to the lives of children, young people and adults with a learning disability and/or autism who display behaviour that challenges, including those with a mental health condition.*

Plans include things like improving community services so that people can live near their family and friends, and making sure that the right staff with the right skills are in place to support and care for people with a learning disability and/or autism.

Each of the local plans will be ‘living’ documents, continuing to be developed in partnership with people with a learning disability and/or autism and their families and carers, as well as charities and other groups, to evidence a real shift in the balance of power and to make sure the plans meet local need and continue to drive up the quality of care.

Swindon and Wiltshire

Swindon and Wiltshire Transforming Care Partnership’s (TCP) plan focuses on prevention and developing a new way of caring for people with complex needs who have a learning disability and/or autism.

The TCP’s plan includes planning for a crisis before it happens, new learning disabilities intensive support services, an emergency pre care and treatment review protocol and the use of local mental health inpatient beds and new specialist flats for people needing to move on from hospital

The plan includes investment in services for people with autism, improving the skills of the workforce and delivering creative options for day activity and short breaks. The TCP will support the development of community assessment and treatment, continuing the use of local hospital options. When low and medium secure placements are needed it plans to work in partnership to deliver options that are closer to home.
